These Terms of Service ("Terms") govern your use of this website and its booking form, operated by Recovery Medical Transportation Inc. ("Recovery Medical Transportation," "we," "us," or "our"). By using this website or submitting a transportation request, you agree to these Terms.

Non-Emergency Service Only

Recovery Medical Transportation provides non-emergency medical transportation only. This website and booking form must not be used for medical emergencies. If you or someone you are assisting is experiencing a medical emergency, call 911 immediately.

Booking Requests Are Not Confirmed Rides

Submitting the booking form, receiving a booking reference number, or paying a booking deposit does not guarantee vehicle availability or confirm your ride. A transportation request is only confirmed once our dispatch team directly contacts you to confirm pickup time, vehicle availability, and other details.

Accuracy of Information

You agree to provide accurate and complete information when submitting a booking request, including pickup and destination details, scheduling, and mobility or assistance needs. Inaccurate information may delay or prevent us from providing transportation.

Please do not submit diagnoses, insurance member IDs, Social Security numbers, payment card information, or other sensitive medical information through the booking form's free-text fields.

Booking Deposits and Payment

Where applicable, a booking deposit is collected through Stripe's secure, hosted payment page. We do not collect or store payment card information directly. Deposit handling, refunds, and cancellations are described in our Cancellation & Refund Policy.

Website Use

You agree not to misuse this website, including attempting to interfere with its operation, submitting fraudulent booking requests, or attempting to access data that is not yours.

No Warranty

This website and booking form are provided on an "as is" basis. While we aim to keep the site available and accurate, we do not guarantee uninterrupted access or that all information on the site is free of error.

Limitation of Liability
